2 HSE Management Organization HSE管理组织机构 HSE management committee will be established for project team which the project manager and site manager will be the chairman. The member of HSE management committee is construction manager, safety

manager,

department

managers

，

construction team leader（engineer），safety engineer, safety inspector. Safety manager, as an assistant of project manager, whose responsibility is supervise and manage the site

safety, housekeeping, environment protection. Safety engineer, safety inspector shall be arranged, Please refer to the attached HSE organization chart.

项目部成立以项目经理和现场经理为首的安全生产领导小组，小组成员有施工经理、安全经理、部门经理和施工队长（工程师）、安全工程师、安全员。项目部设置安全经理，协助项目经理对施工现场安全生产、文明施工、环境保护、职业健康工作进行有效的监督、管理。安全经理下设专职安全员、HSE管理组织机构图（见附图一）。

Monthly meeting will be held by HSE management committee to solve the problem during inspection, to consider countermeasure, to check the un-completed item during inspection of last month and other issues related to HSE. HSE mass meeting will be held by safety manager on every week, all the safety management staff shall take part in the meeting. In the construction peak time, the tool box meeting shall be held every day to solve the problem as soon as possible.

HSE领导小组应进行每月一次例会，会议议程包括检查中发现的问题和要求采取的措施，上月检查未完成的事项和其它与HSE相关的问题。项目部每周一上午举行全体职工参加的安全例会（在施工高峰期每日召开安全碰头会，及时解决安全问题，消除生产安全隐患）。

3.1 Project Manager’s HSE Responsibilities 项

目经理HSE职责

3.1.1 Project manager shall be the major

responsible person for HSE.

项目经理是项目部HSE工作第一责任人。 3.1.2 Project manager shall organize, design,

control and coordinate for the project HSE work.

组织、策划、控制与协调对本项目的HSE管理工作。

3.1.3 Project manager shall establish the HSE

management system, organize the relative person to prepare the HSE management rules to ensure the construction and the housekeeping can be performed in safe condition, so that to reach the HSE managing aim.

负责建立项目部HSE管理机构、组织制定HSE管理制度，确保安全生产、文明施工，达到

预期的管理目标。

3.1.4 Project manager shall arrange enough

cost, manpower and equipment for HSE managing work.

落实项目HSE管理所需的资金、人员、设施等资源。

3.1.5 Project manager shall give the

instruction to carry out the emergency plan while the emergency incident occurred; shall organize the salvage and cooperate for investigation.

发生紧急情况后启动应急救援预案，组织抢救并配合调查。

3.2 Field Manager’s HSE Responsibilities 项目

现场经理HSE职责

Field Manager shall be as assistant of project manager, and he is authorized person to perform the project manager’s HSE responsibilities while project manager is absent.

协助项目经理工作。在项目经理不在项目部时，

履行项目经理HSE职责。

3.3 Construction Manager’s HSE Responsibilities

项目施工经理职责

3.3.1 Construction manager shall response for

the site HSE managing work under project manager’s leading.

在项目经理的领导下对施工现场的HSE管理工作负责。

3.3.2 Construction manager shall response for

the HSE plan organization and execution. Construction manager shall make the schedule, comparison

arrangement, for

inspection,

work,

summarizing and appraising through

construction

meanwhile, the same work shall be done for HSE by construction manager.

组织实施HSE管理规章制度，在计划、布置、检查、总结、评比生产时，同时计划、布置、检查、总结、评比HSE工作。

3.3.3 Construction manager shall push the

relative person for the safety potential

dangerous and stop the infringed work. 督促安全隐患的整改，制止违章作业。 3.3.4 In case the accident occurred,

construction manager shall give the instruction to protect the accident area, shall

participate

investigation, and shall take action of countermeasure.

3.4 TCC Safety Manager’s Responsibilities TCC

安全经理职责

Safety manager shall supervise and manage the site construction safety, housekeeping, environment protective, and occupation health work under project manager’s leading. He also shall ensure all site work activities meet the requirement on health, safety and environment protective by EPCL PVC, CNCEC and local government. HSE inspection shall be organized and executed by safety manager.

Suitable HSE plan, rules shall be prepared by safety manger. Safety manager also will supervise the PPE issuing and usage. In case the accident occurred, safety manager shall perform the investigation and handling. Safety manager is authorized to stop the work under the dangerous condition.

在项目经理的领导下，负责对施工项目安全生产、文明施工、环境保护、职业健康工作进行有效的监督和管理，保证在现场进行的全部工作符合业主、公司、当地地方政府对健康、安全、环保的标准要求。组织进行HSE检查，制定适合项目的HSE管理制度、管理方案，监督劳动防护用品的发放和使用。组织事故调查和处理。在危险危急情况下有权发布停工令。

3.5 CC-7 Safety Manager’s Responsibilities

CC-7安全经理职责

To implement the HSE procedure strictly and confirm responsibilities of safety engineer and inspectors, and they should be supervised and managed effectively. You should ensure to

implement the safety technology during the construction .To organize HSE checking. And he shall negotiate with other departments to make sure the specific person to supervise and rectify the hidden hazards. Meanwhile, it’s necessary to investigate accidents and issue the order of stopping work under the emergency condition.

严格有效执行安全管理体系，明确安全工程师和安全员的职责，并对其进行有效的监督和管理。组织HSE检查，确保施工过程中安全技术措施的落实。与项目其它部门进行有效协调，明确隐患整改人，督促安全隐患及时整改。组织事故调查处理，在紧急危险情况下，有权发布停工令。

3.6 Safety Engineer’s Responsibilities 安全工

程师职责

The safety training engineer is responsible for the training of Chinese newcomers and special type of work, and educates them to redress the incorrect operation; the training of emergent rescue and the management of

safety document .Safety engineer for scaffoldings is responsible for checking the design of scaffolding. Before using the scaffoldings, the safety engineer should hang the tag and supervise to rectify the hidden hazard. Safety engineer for instruments is responsible for checking and posting the equipments at the site and to rectify the hidden hazard.

安全培训工程师负责全场新进中国人员的入场培训，特殊工种的专项培训，违章操作再教育，紧急救援培训及安全文件资料管理。专业安全工程师负责对脚手架搭设方案审核，脚手架搭设完成后挂牌验收，督促脚手架隐患整改。专业安全工程师负责现场机具，用电设备检查贴标，督促机具设备隐患整改。

3.7 Safety inspector’s Responsibilities 专职

安全员职责

Safety inspectors shall response for the safety manager as assistant under safety manager’s leading. Safety inspectors shall

supervise for the execution of the HSE managing rules and safety measurement in their own area. Safety inspectors shall participate the HSE inspection. He can stop the infringed work. Safety inspector is authorized to stop the work under the dangerous condition, and he will report to safety manager timely. Safety inspector shall study the HSE managing development and submit the suggestion and measurement for improvement.

participate or organize the inspection work for related safety protective facilities. Safety inspector also will supervise the PPE issuing and usage. Monthly statistic, analyze and report for HSE managing shall be performed by safety inspector. The area safety inspector should insist on cruising, supervising and make relevant records everyday.

3.8.3 Safety clarification shall be given to

worker by engineer. And it must have written records.

进行安全技术交底，交底应有书面记录。

3.8.4 Construction engineer shall supervise

the workers complying the construction method and operation rules. Construction engineer can stop the infringed work. 督促作业人员严格执行施工工艺及操作规程，制止违章作业。 3.8.5 Construction

He will response for the rectification work regarding the potential dangerous of construction temporary electrical. 电气技术人员除了履行以上职责，还应履行：a. 编制“临时施工用电方案”； b. 参加每月一次施工用电安全检查，并负责对施工用电安全隐患的整改。

3.8.8 The technicians and captain must apply

to the area safety inspector before they are ready to work under the safe condition. The detail can be referred to the requirement for work permit.

工程技术人员、队长在进行作业许可规定的作业前，必须向区域安全员申请，区域安全员在保证安全措施的前提下，进行批复后才可作业。具体参见作业许可规定。

3.9 Worker’s HSE Responsibilities 作业人员

HSE职责

Worker shall strictly comply with the company and project team’s HSE rules. Worker shall perform the construction work as per safety requirement. Worker can stop the infringed work and refuse the infringed instruction. Worker shall carry out the housekeeping, comply the HSE managing and command. Any accident person.

严格遵守公司、项目部制定的HSE管理规章制度。严格按安全规程实施作业，有权制止违章作业，有权拒绝违章指挥。文明施工，服从HSE管理和指挥。发现事故或事故苗头，立即报告上级有关人员。

4 HSE Management Principle 项目HSE管理原则 “Human shall be the root. Safety First. Prevention is the major Measurement.” is principle of HSE management. “Anyone who response for construction, he will also response

项目HSE管理必须坚持以人为本，安全第一，预防为主的方针和管生产必须管安全的原则。

“Safety First”, its means that safety work is the primary important work and the priority work during construction. “Prevention is the major Measurement.” its means that the relative countermeasure shall be taken against the potential dangerous while arranging construction work. Construction work cannot be carried out unless the safety protective measurement has been executed.

安全第一，就是要求在进行施工活动时，把安全工作放在一切工作的首要位置，实行安全优先的原则。预防为主，就是要在安排、布置施工任务时，针对施工中可能出现的危险、危害因素，采取预防措施予以消除，实行安全防护措施不落实不开工的原则。 Falling down from height, hitting by object, injury by machine, collapse and electrical shock

is the most likely occurred accident due to the site condition is complex with large quantity worker, construction machine and material, cross work by multi-discipline, some work at opening area, various working condition, manpower and machine position changed frequently. Therefore, to control the un-safe activity of manpower and un-safe status of object is the key point of safety managing. “Human shall be the root” shall be kept in everybody’s mind. HSE shall be controlled by everybody, in everywhere, during whole process. To create a good safety construction condition, and to reach the HSE management aim, the relative resource shall be cost. The relative resource include professional safety inspector arrangement, doctor, first aid point, first aid box and scaffolding arrangement, plank placing, fire extinguisher arrangement, PPE provision and safety net installation, etc.. HSE plan will be arranged and checked according to the construction schedule. Safety manager will

attend the coordination meeting between EPCL PVC and CNCEC for the safety issues coordination. 施工现场作业人员密集，施工机械、材料集中，多任务种立体交叉作业多，露天作业多，作业环境多变，人员、机具流动性大，高处坠落、物体打击、机械伤害，坍塌、触电是容易发生的事故，控制好人的不安全行为和物的不安全状态是安全管理的重点。必须坚持“以人为本”的管理思想，进行全员、全方位、全过程的动态控制，调动全体人员参与HSE管理，形成良好的安全文化氛围。要创造一个良好的安全环境和氛围，实现HSE管理目标，必须投入相应的资源。这些资源包括：配备专职安全员，配备专职医生和急救站，急救箱，脚手架，跳板铺设，消防灭火器的配备，PPE配备及安全网铺设等。施工HSE计划随着施工计划进行安排和检查。安全经理将出席EPCL PVC和项目部协调会，在会上对HSE信息进行沟通。

5 HSE Responsibilities Execution and Management 落实HSE责任制，实施责任管理

The core of HSE responsibilities principle is to achieve “5 safety work (make the schedule,

arrangement, inspection, summarizing and

appraising through comparison) shall be done while executing the 5 same work for construction.”. Each subcontractor shall sign for the safety construction agreement with CNCEC project team (or the safety construction and housekeeping requirement included in the contract), so that, the safety construction and housekeeping can be effected by cost. The risk cost for safety construction and housekeeping shall be 5% of total amount of subcontract. If the subcontractor cannot reach to the safety and housekeeping target instructed by CNCEC, then the 5% risk cost shall be deducted. In case some infringed thing was discovered (such as construction

material

placed

everywhere,

construction rubbish cleaning un-timely, safety net arrangement isn’t meet the requirement, air break and flammable explosive material placing isn’t meet the requirement), rectification is required timely; otherwise, 1% of total amount of

safety and housekeeping risk cost shall be deducted for once.

安全生产责任制，其核心是要实现安全生产的“五同时”：在计划、布置检查、总结、评比的时候，同时计划、布置、检查、总结、评比HSE工作。各分包单位与项目部签定安全生产协议（或承包协议书上必须有安全、文明施工指标），使安全生产、文明施工与经济责任制挂钩。安全生产、文明施工风险金占分包合同总额的5%，分包单位未完成项目部下达的安全、文明施工指标，扣除责任单位5%风险金。施工材料乱堆乱放，建筑垃圾不及时清理，不按规定设置安全网，漏电保护器，易燃易爆物放置不符合规定等，经指出的不及时整改的，发生一次扣安全生产、文明施工保证金总额的1%。

6 HSE Education & Training HSE教育与培训 6.1 All the personnel to be mobilized shall take

part in safety training and education organized by Owner and project managing department. One professional safety engineer shall be designated for HSE education and

training work. HSE training shall be given to all person who will enter the site. More safety training and education such as safety requirement for exactly work and safety operation procedure shall be given by foreman before starting the work. EPCL shall response for training and education for local worker. Proper PPE, ID card, uniform shall be issued to workers. Anybody who perform the work infringed the rules, shall be given safety training again in current afternoon to improve his

safety

consciousness.

Everybody

(including infringed person) shall register after safety training for record.

项目所有进场人员，除参加业主的安全培训和教育外，还要参加项目的安全教育。项目部设一名专职安全师负责对职工进行HSE教育、培训工作，进入施工现场的人员，必须经过项目部安全培训；到作业班组后由班组长再进行本岗位安全注意事项和安全操作规程等培训教育。配备PPE，统一工作服，持工作卡上岗。应特别注意对当地雇用

民工，由业主进行安全教育和培训获得入场资格同时按CNCEC要求，配备统一工作服，PPE并颁发工作卡后才能入场工作。现场违章人员，于当天下午接受安全教育培训，提高安全意识。职工经培训教育后登记备案，对违章人员要进行登记。 HSE Education and Training Content for New Person.

6.1.3 Emergency Plan In Case Accident Occurred

事故应急救援。

6.1.4 CNCEC will arranged three sets of TV and

one set of DVD to display some movies and

documents regarding the safety issue for giving staff and worker various safety training. Projector shall be used to playing the safety education PPT for the persons which shall enter the site and participate in special construction.

项目部配备三台电视机，一台DVD播放机，可以从国内购买一些安全宣传电视节目和安全宣传资料，准备入场安全教育，特种作业人员安全教育，专项培训等PPT，用投影仪进行播放，对作业人员进行形式多样的安全教育培训。

7 HSE Award and Punishment HSE奖励与处罚 HSE award and punishment rules shall be established for site construction in order to strengthen occurred.

为了加强安全生产、文明施工管理，落实安全、文明施工责任制，有效防范重大伤亡事故的发生，文明施工，特制定施工现场安全生产、文明施工奖励与处罚

8.1 HSE Award 奖励

8.1.1 HSE Check List shall be:

a.HSE managing system, standard, rules and quality managing measurement are all perfect.

b.Death / GBH Accident=0 c.Occupational Disease=0

d.Environmental Polluting Accident=0 e.Fire / Explode Accident=0

f.Property loss Accident which value is upper than RMB50000=0

g.The recorded accident ≯1 (per 200000MH) If there has no hereinbefore item happens, the safety bonus shall be awarded monthly. 8.1.1 HSE考核指标如下：

a. HSE管理体系完整，制度标准健全，质量管理措施落实到位； b. 死亡/重伤事故=0； c. 职业病=0； d. 环境污染事故=0；

e. 火灾/爆炸事故=0；

f. 超过5万元的财产损失事故=0； g. 可记录事故≯1（每20万人工时）。

全部达标后，将按月发放安全奖。具体额度现场制定。

8.1.2 In case “Zero Accident” target is

reached in every three months, every half an year, every year, every 500 thousand man-hours, every 1 million man-hours, some activities will be held for celebration. The details shall be decided by HSE management committee.

每季度、每半年、每年、每50万工时、每100万工时施工无事故，项目部将适时组织参战职工进行庆祝，发纪念品或奖金。详细活动将由HSE领导小组讨论决定。 8.1.3 HSE award shall be given to some persons

for each construction team in every month. The percentage of award persons is 2%. HSE领导小组每月在现场评出安全、文明施工先进个人，人数占总人数5%，给予物质奖

励，并通报表扬。

8.1.4 Some person who comply the safety

requirement, stop the infringed work timely and avoid the serious safety accident, will be awarded.

对于在施工中遵章守纪，及时制止违章作业，避免了重大安全事故发生，项目部给予物质奖励，并通报表扬。

8.2 Punishment for the Infringed Activities 违

反HSE管理制度的处罚

8.2.1 Minor Infringed Activities 以下行为

属轻微违章行为

a. Don’t wear PPE or work cards as per requirement; 未按规定穿戴个人防护用品或未佩带工作卡；

b. Smoking at site non-smoking area; 在现场非吸烟处吸烟；

c. Listen music at site; 在现场听音乐； d. Sleeping at site (including in vehicle); 在现场睡觉（包括在汽车驾驶室） In case the minor infringed activities

discovered, warning shall be given for first time. If it is discovered again within 30 days, then the infringed person will attend the training and registering and 10 USD punishment shall be given.

凡发生轻微违章，第一次警告，若30天内再次违章，组织学习、登记备案，并给予10美元的经济处罚。

8.2.2 Serious Infringed Activities 以下行

为属严重违章

a. Don’t use harness while working at height; 高处作业未正确系挂安全带； b. Relieve oneself at site everywhere; 在现场随地大小便；

c. Throwing down tools or material from up; 从上往下抛工具或其它物体；

d. Driving without license & driving irresponsible;

无证驾驶机动车，不负责任地驾驶机动车； e. Without work permit as per requirement;

要求办作业许可证的地方未办理作业许可证；

f. Damaging safety equipment or using safety equipment doesn’t meet the requirement.

损害或滥用安全设施、设备。

In case the serious infringed activities discovered, it shall be record and the infringed person will attend the training again and 50 USD punishment shall be given.

凡发生一次严重违章，记录在案，组织学习，并给予50美元的经济处罚。

8.2.3 In case the fighting, bust-up, gambling,

purposive damaging, steal construction property or equipment discovered, it shall be record, the infringed person shall attend the training again and 150 USD punishment shall be given. If anybody ignore the training and education, the infringed activities occurred again, then

200 USD punishment and necessary administration punishment shall be given, and this person shall be sent back to China.

凡发生打架斗殴、聚众赌博，故意损坏、偷盗现场财产、设备的，登记备案，组织学习教育，处罚150美元。对于不听劝阻、教育、继续发生打架斗殴、聚众赌博，故意损坏、偷盗现场财产、设备，给予200美元经济处罚和必要的行政处分，并遣送回国。

8.2.4 对管理人员违章的处罚标准

Punish Policy for the Peccancy Managing Staff

a. 管理者违反安全规章制度、标准，强行指挥工人作业者，每次对责任人罚款100美元，并将此违章管理者清出现场。

The person that forces workers to performing works against the safety rules and standard will be kicked out of the site and amerced USD100.

b.脚手架、安全网及井架、门架、起重设备等，未经检查验收就投入使用的每次对责任人进行处罚50美元。

Only checked and qualified scaffold, safety net, gantry and lifting equipments can be used. If someone act against this rules, the person responsible shall be amerced USD50.

c. 使用不合格的机械设备或未贴合格证就投入使用，每次对责任人进行处罚20美元。 Equipments without certificates and unqualified equipments can not be used. If someone act against this rules, the person responsible shall be amerced USD20. d.不对班组进行安全交底者，每次对责任人罚款50美元。

Safety clarifications shall be done to the construction team and group, if someone

act against this rules, the person responsible shall be amerced USD50. e.接到隐患整改通知书逾期不整改者，对责任人进行处罚50美元。

Modifying work shall be done as soon as possible while receiving the Modification Notice, if someone act against this rules, the person responsible shall be amerced USD50.

f.“四口、五临边”无安全防护措施或措施不到位者，每发现一处罚责任人50美元。 If there has un-perfect or no safety protection

measurement,

for

each

unqualified item, the person responsible shall be amerced USD50.

g.现场配电箱不符合标准或接线不符合规范者，每发现一处，罚责任人20美元。 If the site distribution boxes and terminations are not meet requirements,

for each un-qualified item, the person responsible shall be amerced USD20. h.无许可证作业者，对责任人进行罚款50 美元。

If there has someone working without permit, the person responsible shall be amerced USD50.

i.对现场安全管理人员指令拒不执行者，对责任人处罚50美元，相关责任人清出现场。 If there has someone inexecuting safety inspector’s

command,

the

person

responsible shall be amerced USD50 an the relative person responsible shall be kicked out of site.

8.2.5 For local employees, we will give them

safety education in case the infringement noticed. If the serious infringement is noticed after safety education, then, they will be dismissed immediately.

对当地雇用民工，我们发现他们违章，应进行安全教育，如果经教育后仍然严重违章，CNCEC将对该雇员予以解雇。

8 Safety Countermeasure and Safety Clarification 安全技术措施和安全技术交底

The corresponding safety countermeasure (JSA job safety analysis), shall be included in the construction method statement. The special safety countermeasure plan shall be prepared for the following special work or work that with more dangerous. It can be executed after approved by project manager.

要求工程技术人员在施工方案中，编制有针对性的安全技术措施，进行JSA分析，即作业安全分析。以下危险性较大的作业、特殊工程施工（作业），必须制定单独的安全技术措施方案，并经项目经理部逐级批准后执行。

9.1 Pit Support and Dewatering Work 基坑支护与

降水工程；

9.2 Earth Excavation Work 土方开挖工程；

9.3 Form Work 模板工程； 9.4 Slip-form Work 滑模工程；

9.5 Big Size Scaffolding Work ( scaffolding above

20 meters, loading scaffolding, mobile scaffolding and special scaffolding) 大型脚手架工程（10米以上的脚手架、承重脚手架、移动脚手架和特殊脚手架）；

9.7 Acid Cleaning & Oil Cleaning Work 酸洗、油

洗作业；

9.8 Working in Confined space 进入密闭容器作业 For the special work, safety clarification shall be performed to construction team by construction engineer in written type before start the work. The relative dangerous, harmful and the necessary countermeasure

特殊作业前，应由工程技术人员向班组作书面安全技术交底，告知与特殊作业活动有关的危险、危害，应采取的安全防护措施等。

Tool box meeting shall be held everyday by foreman before start the work. Safety clarification regarding the work to be performed in current day shall be given in tool box meeting. PPE using condition shall be checked. And the work cannot be started if without proper PPE.

每天作业前，由班长进行班前安全例会，针对当天作业进行安全交底，作业班长检查安全防护措施的落实情况，发现问题且不能处理，应停止工作，立即向区域安全员报告，坚持安全防护措施不落实不开工的原则。

9 Requirement for Work Permit 作业许可证规定 Work permit shall be applied before start the work that the height exceed 10m, Scaffolding work and dismantling ，deep pit excavation work with the depth is more than 2.5m (in the middle and end of the construction period, the depth shall change to 1m) , Before starting these works such as working in defined space, working in flammable & explosive area, performing of Crane work, oil

cleaning & acid cleaning work, radial work and slip-form work, the work permit shall be obtained and approved by HSE department.

凡高度在10米以上作业；脚手架搭设和拆除作业；大于2.5M深基坑开挖作业（至工程中、后期，大于1M深度的开挖作业）；进入密闭容器作业；进入易燃易爆危险区域作业；使用吊车进行起重作业；油洗、酸洗作业；射线作业；滑模作业。作业前必须办理作业许可证，经区域HSE部门审批后方能进行作业。 Safety manager shall according to site condition to define whether a certain work area already change to dangerous or not and whether the work permit is required for this area or not. 根据施工现场进展情况，由安全经理决定什么时候现场区域变得危险，并要求采取工作许可证来实施作业。 10 HSE Inspection HSE检查

CNCEC will hold HSE inspection in every week. Safety manager will response for this work, construction manager, safety engineer and construction captain（engineer） will attend the

HSE inspection. Safety inspection for

construction temporary electrical and lifting gear shall be held aperiodically by CNCEC. Safety inspection for fire fighting shall be held in every three months. Portable electric tools, machine and equipments, temporary power supply facilities, PPE and vehicles shall be checked termly. Except this inspection, special inspection and irregular inspection shall be arranged according to site condition to discover and solve the potential dangerous timely and to avoid accident.

项目部每周组织一次HSE检查，由安全经理组织，参加人员为施工经理、各施工队负责人、专职安全员。项目部不定期组织一次施工用电安全检查，吊装索具检查，每季度组织一次消防安全检查。安全工程师、安全员应做好定期对手持电动工具、机械设备、临时用电设施、劳保用品使用、车辆等进行的检查工作。此外，还要根据施工现场实际情况，安排特殊专项检查、突击性检查等，及时发现、处理、清除危险危害因素，避免事故发生。

11 Equipment Control 设备管理

The equipment that to be used at site, shall be with completed and effective safety protective device. The un-acceptable equipment cannot be used at site. Equipment controller shall response for the equipment inspection, managing and maintenance to ensure the equipment in good condition. Using and maintenance record shall be prepared and filed well. After tagged by safety engineer for equipment ,the it can be used. 进入施工现场的设备，必须保证各项安全保护装置齐全、有效，不合格不得进入施工现场。所有进场设备必须经负责设备检查的安全工程师，检查贴标后，才能投入使用。由设备管理员负责对进场设备的检查、管理、维护、保养，确保设备完好，并做好使用、维护、保养记录并存档保存。 12 Lifting Work 起重吊装作业 13.1 Safety Load 安全工作负荷

Safety load means the maximum load can be lifted by crane within the specified lifting

radius.

安全工作负荷是指在一个规定的起重半径内能够被起重机安全操作的最大负荷。

13.2 All crane and lifting machine shall be

inspected as per requirement before using. Checking list shall be submitted to safety manager.

所有起重机和施工设备，在进入现场之前，应按规定程序进行检查，合格后才能进入施工现场，并向安全经理提交起重机检查清单。

13.3 Procurement section equipment controller

shall maintain the lifting machine and prepare the maintenance record to ensure the lifting machine in good condition.

设备材料部机具管理员作好起重机械的维护、保养工作，使起重机械处于良好的工作状态，做好维护记录。

13.4 All wire rope and lifting gear shall be

inspected by lifting engineer before move-in site. It shall be marked after inspection and acceptance for safety working load.

负责吊装工程技术人员应对进入现场的所有起重机钢丝绳和吊具进行检查，并在检查合格之后在上面进行标识，标明安全工作负荷。

13.5 Method statement for heavy lifting shall be

prepared by lifting engineer for the heavy lifting that exceeds 20T. It shall be complied after approval.

20T以上的大件吊装作业，必须由吊装技术人员编制吊装方案，经审批后执行。

13.6 Lifting work permit shall be applied for the

crane work. The work permit shall be issued by area safety inspector.

起重机作业时，必须办理吊装作业许可证，许可证由区域安全员签发。

13.7 Warning tape and warning sign shall be

provided for lifting area. Unnecessary person cannot enter the lifting area.

吊装区域设置警戒线和警示标志，无关人员不得入内。

13.8 Safety technical clarification shall be

given to worker by lifting engineer before

start the work.

吊装作业前必须由吊装技术人员向作业人员作好书面安全技术交底。

13.9 An authorized person shall be arranged for

lifting work to give the lifting instruction. All person participate the lifting work must follow this authorized person.

吊装工作必须设一个吊装总指挥，所有吊装指令由总指挥下达，所有参加吊装作业的人员必须服从吊装总指挥的命令。

13 Working at Height 高处作业管理

14.1 The person who has any disease that not

suitable for working at height (such as hypertension, heart disease and epilepsy etc.), cannot be allowed working at height. 有高处禁忌症者（如高血压、心脏病、癫痫等）不得从事高处作业。

14.2 PPE shall be used properly (helmet, harness,

uniform and antiskid shoes).

正确使用个人安全防护用品（安全帽、安全带、

工作服、防滑鞋）。

14.3 Working on the infirmly or unsafe position of

temporary building, glass sunshade, balcony edge, asbestos sheet shall not be allowed. 不得站在临时搭设的建筑物、玻璃天棚、阳台口、石棉瓦等不牢固的结构或不安全的位置上进行操作。

14.4 No passing shall be allowed while person

working at height. If it is necessary, then the safety isolation shade to be installed. 高处作业者下方应禁止有人行走，不可避免时应搭设安全隔离棚。

14.5 While the working placing is near the high

voltage power cable, then the work can be performed

after

using

the

countermeasure.

临近带电的电源线高处作业时，必须采取可靠的安全措施后方准作业。

14.6 Throwing down any tools, material or rubbish

during working at height shall not be allowed. 高处作业时不准向下抛掷任何工具、材料、垃圾

等物品。

14.7 While working at height, the material shall

be placed firmly. Tools shall be placed in the tool bag or special tool box.

高处作业所使用的材料应堆放平稳，工具应妥善地放在工具袋内或特设的工具箱内。

14.8

While working on the pipe rack or frame, if worker shall walk along the single beam, then the horizontal

life line shall be set. Harness shall be hung on the life line. 在管廊架或框架上作业时，如必须沿单梁行走，要设置水平安全绳，作业人员应将安全带挂在安全绳上。

14.9 While working at height, handrail shall be installed around the

platform.

高处作业平台四周要设防护栏杆。

14.10 If there is any opening in the working area, then it shall be covered

by firmly plate or barricaded by 75cm net.

对于作业面处的孔洞，开口处应设置牢固的覆盖板或高度为75cm的围栏。

14.11 While working on the grating platform, then the grating shall be full

placed and fixed. If the grating shall be removed cause of construction requirement, then safety net shall be set around the opening area, and the removed grating shall be re-installed timely. 在格栅板式操作平台上作业，格栅板必须满铺并固定，如因施工原因拆除格栅板，应设置围栏，并及时将格栅复位。

14.12 The protective shade shall be installed for the entrance of the

building that under construction. The length of protective shade shall be 3~6m, the width of protective shade shall be extended on

both sides of entrance for 1m. Wooden plate shall be placed on the top of shade. Both sides of entrance shall be enclosed as well as some non-entrance opening.

正在建设中的建筑物的通道入口应搭设长3－6M，宽于出入道两侧各1M的防护棚，棚顶应铺满脚手板，非出入口和通道两侧必须封闭。

14 Requirement for Hot Work 现场动火管理规定

15.1 Oxygen and acetylene cylinder shall be with completed safety

accessories and safety device.

氧气乙炔瓶必须要有完善的安全附件和安全装置。

15.2 Oxygen and acetylene cylinder cannot be placed under the sun shine

directly. The distance between oxygen cylinder and acetylene cylinder shall be not less than 5m. Oxygen cylinder and acetylene cylinder shall be stored apart from fire at least 10m. Acetylene cylinder and oxygen cylinder shall be vertical placed during storage and usage.

严禁氧气、乙炔瓶在阳光下爆晒。乙炔瓶、氧气瓶之间的距离不得小于5M；乙炔瓶、氧气瓶的存放必须距离明火10M以上。乙炔瓶，氧气瓶储存和使用应立放。

15.3 All flammable and explosive material near the hot work position shall

be removed before start the hot work. While hot work shall be carried out at opening area such as using pit furnace, making asphalt), then it shall be located apart from flammable material warehouse at least 25m.

在动火作业前，一定要把动火地点的一切易燃、易爆物清理干净，露天动火（如生地炉、熬沥青）时距离易燃物品仓库的安全距离不得小于25M。

15.4 Fire extinguisher shall be provided according site condition, and

fireproof countermeasure shall be adopted.

应根据实际情况配置好灭火器材及采取有效的防火措施。

15.5 After hot work completion, site shall be checked, worker can leave

the site after slaking the ash fire, shut down power and confirming site without potential dangerous.

动火作业人员工作完毕后必须检查现场，熄灭残余火、切断电源，确认无安全隐患时方可离开现场。

15.6 If hot work shall be carried out on the pipe or equipment with

flammable and explosive gas or liquid, then the gas or liquid shall be cut; valve shall be closed; blind flange shall be installed. Hot work can be carried out after replacement and acceptance. Welding grounding cable connected on the pipe and equipment shall be strictly prohibited.

凡在易燃易爆的气体或液体管道上、设备上动火，都必须先切断气体、液体，关闭阀门、插上盲板，经置换合格后方准动火。严禁电焊把线接地线接在管道和设备上。

15.7 While hot work shall be executed inside of equipment, vessel, pipe

and trench, inspection is required before start the work. Meanwhile, watchman is required to arranged out side of it. Attention shall be paid, gas mask is required if working shall be performed in the vessel that has been filled with poisonous medium before.

在设备、容器、管道、地沟等内部动火，必须经检验合格后方准动火，同时还必须安排监护人员在外面进行监护。如进入盛装过有毒介质容器内作业，必须携带防毒面具。

15.8 Harness shall be used while hot work shall be performed at height.

If there is any flammable or explosive material or equipment, it shall be removed or isolated before start the work.

高处动火作业必须拴好安全带，若周围有易燃、易爆物质或易燃、易爆设备时，应有专人进行清除或采取隔离措施。

15.9 Work permit shall be applied for hot work in flammable and explosive

area. And related safety protective countermeasure shall be provided before start work.

凡在易燃、易爆危险区域内动火，必须办理《动火许可证》，并切实采取了安全防护措施方能动火。

15 Procedure for Working in Confined space 封闭空间准入规定

1 Aim 目的

This procedure is applied for ensure the safety of worker working in confined space and to avoid the un-necessary person enter the confined space without permit.

本程序的目的，在于保障进入封闭空间员工的人身安全，并防止未经许可的人员擅自进入封闭空间。 2 Definition 定义

Air inside the confined space shall be defined in three kinds. 封闭空间内部的空气分为以下三类： 2.1

Normal Air: Normal air means the content of oxygen is 21%, the content of toxic gas, liquid and powder shall be less than lower limit, the content of flammable gas shall be lower than 5% of the flammable lower limit.

正常空气：正常空气是指氧气的体积含量为21%，毒性气体、液体粉尘的含量低于下限，可燃气体的含量低于可燃下限的5%。

2.2 Abnormal Air: Abnormal air means the content of oxygen is out of

19.5%~22%, and the content of toxic gas, liquid or powder is higher than allowance value, the content of flammable gas is higher than 5% of flammable lower limit.

异常空气：异常空气是指氧气的体积含量不在19.5%－22%之间，而且毒性气体、液体或粉尘的含量高于允许值，可燃气体的含量高于可燃下限的

5%。

2.3 Inactive Gas: Means the air including inactive gas, so, the content

of oxygen is lower than 16%.

惰性气体：空气中有惰性气体，故氧气的含量低于16%。

3 Applicable Scope 适用范围

This procedure is applied for all work activities in confined space such as oil box, pipe, drainage pipe, pit that depth more than 1.2m. 本程序适用于进入所有的封闭空间，如油箱、管道、下水道，深度超过1.2m的深坑。

4 Procedure 程序

1.1

Method statement for working in confined space shall be prepared. The relative safety countermeasure shall be included in the method statement. Written type safety clarification is required. 工程技术员必须编制封闭空间施工方案，方案上要有针对性的安全措施，并进行书面安全交底。

1.2

Periodic inspection for oxygen and other gas content shall be performed before enter the confined space. For example working in every other day.

进入封闭空间作业，都要进行氧气和其它气体的定期检测（如时隔一天后作业）。

1.3

At least two person working in confined space shall be allowed. Watchman shall be arranged outside of the confined space. Watchman cannot leave the working place before worker in confined space moving out.

进入封闭空间必须两人以上作业，任何人不得单独作业，并且必须在封闭空间以外设有监护人。监护人在作业人员未出来这前不得擅自离开。

1.4

Harness and life line shall be used for the worker in confined space. Worker in confined space shall contact with watchman anytime.

进入封闭空间作业人员必须拴好安全带和安全绳，并随时与监护人联系。

1.5

The lighting voltage used in confined space under the dry condition shall be 24V, it shall be 12V under the wet condition. Portable light transformer cannot be used in the confined space. The individual socket with air break switch shall be provided for each electrical machine.

封闭空间内的照明电压在干燥环境中为24V，潮湿环境下为12V。行灯变压器不得带入容器内。每个电动设备都应该有各自的电源插座，并有漏电保护器。

1.6

Fan shall be provided for confined space to lead in the fresh air. 进入容器作业应该布置鼓风机或引风机，引入新鲜空气。

1.7

Work permit shall be applied for working in confined space. It shall be issued by CC-7 safety manager.

进入封闭空间必须办理作业许可证，由CC-7安全经理签发作业许可证。

1.8

Oxygen, acetylene, nitrogen cylinder etc. cannot be taken into the confined space. If the gas welding must be used in confined space, the oxygen cutter and cable shall be taken out of the confined space after gas welding completion to avoid leakage.

氧气瓶、乙炔瓶、氮气瓶等其它气瓶不得带入封闭空间内。如果需要动气割，气割完应将割把移出封闭穴，以免泄漏。

16 Flammable & Explosive Material Control Rules 易燃、易爆化学危险品的管理规定

16.1 Flammable & explosive chemical material shall be handled slightly.

Impacting, pulling or dumping shall be prohibited. 轻拿轻放，防止撞击、拖拉和倾倒。

16.2 Some chemical materials that maybe creating burning, explosive or

other dangerous in case impacting or contact each other, it cannot be transported together and it shall be transported as per

requirement.

碰撞、互相接触容易引起燃烧，爆炸危险的化学危险品，不得违反配装限制和混合装运。

16.3 Some chemical materials that maybe creating burning or releasing

poisonous gas in case under the hot or wet condition, then, some countermeasure for heat insulation or damp-proofing shall be adopted during transportation.

遇热、遇潮容易引起燃烧或产生有毒气体的化学危险品，在装运时应当采取隔热、防潮措施。

16.4 The flammable and explosive chemical material shall be registered

before storage. The periodical inspection shall be performed after storage.

易燃、易爆化学危险品入库前，必须进行检查登记，入库后应当定期检查。

16.5 The flammable and explosive chemical material shall be stored

separately in special warehouse. The warehouse shall be cool, dry and with good ventilation.

易燃、易爆化学危险品必须设立专用仓库分别存放，并保证库房的阴凉、干燥、通风。

16.6 Smoking and using fire in chemical material storage warehouse shall

be strictly prohibited. Fire-proofing countermeasure shall be adopted for the vehicle that will enter the warehouse.

储存化学危险品的仓库内严禁吸烟和使用明火，对进入仓库内的机动车辆必须采取防火措施。

16.7 The related countermeasure such as ventilation, fire-proofing,

explosion protection, gas protection, flashback device, lightning protection and antistatic shall be adopted while using the flammable, explosive and chemical material according to the material type and performance.

易燃、易爆、化学危险品使用，应根据其种类、性能，采取相应的通风、防火、防爆、防毒，防止回火、避雷、防静电等安全措施。

16.8 Identification shall be provided for the chemical material at site.

对现场的化学危险品进行标识。

17 HSE Control for Construction Electrical 施工用电安全管理

17.1 “Method Statement for Temporary Construction Electrical” shall be

prepared by electrical engineer, and it can be applied after approved by project manager.

现场施工用电，必须由电气工程师编制《施工现场临时用电方案》，并经项目部批准后执行。

17.2 Rain-proof device shall be provided for all site electrical

distribution box, switch box and electrical equipment. All electrical distribution box, switch box and electrical equipment shall be with good ventilation and dry condition.

施工现场的所有配电箱、开关箱及带电的设备，必须设防雨保护装置，并设置在通风干燥的地方。

17.3 The damaged and unacceptable cable and machine cannot be used. If

any damage of cable or plug is discovered during construction, it shall be replaced timely.

严禁使用破损、不合格的电缆、电气设备。施工中发现破损的电线、插头，维修电工应及时更换。

17.4 One switch and one air break switch shall be provided for each machine.

Air break switch shall be provided for main distribution box, sub-distribution box and switch box.

施工现场用电实行“一机、一闸、一保护（漏电保护器）。总配电箱、分配箱、开关箱都要安装漏电保护器。

17.5 Lightning countermeasure (grounding or using lightning arrester)

shall be provided for tower crane, gantry or high scaffolding. 塔吊、龙门架、高大脚手架必须有防雷击的措施（接地或安装避雷针）。

17.6 Grounding shall be provided for some metal shell of electrical

equipment such as welding machine. 电焊机等电气设备金属外壳必须作接零保护。

17.7 Electrical distribution box shall be locked and control by designated

person. The equipment name shall be marked on each switch in the distribution box.

配电箱门应配锁，由专人负责。开关箱内每只开关必须标明用电设备名称。

17.8 Wiring work for construction electrical only can be performed by

professional electrician.

现场施工用电接线必须由专业电工进行，其余人员不得私自接线。

17.9 Safety inspection for construction temporary electrical shall be

held once a month by CNCEC.

项目部每月组织一次施工用电安全检查。

17.10 All electrical equipment shall be checked by CNCEC’s electrical

engineer.

现场所有的临时施工用电设施都要经CNCEC电气工程师检查确认。

18 Scaffolding Work 脚手架

18.1 Scaffolding shall be installed by scaffolding installation group.

Scaffolder with qualification certificate can be allowed to execute the work.

现场所有脚手架的搭拆，均由项目部脚手架施工队进行。架子工应持证上岗。

18.2 Method statement shall be prepared by scaffolding technician for

large-scale scaffolding installation and dismantling. The scaffolding installation and dismantling work can be carried out as

per method statement after it approved by construction manager. Work permit shall be applied for scaffolding installation and dismantling.

大型脚手架安装和拆除，必须由技术员编制施工方案，经现场总工程师审核，施工经理批准后执行。拆除脚手架时必须办理作业许可证。

18.3 For the scaffolding, after installation, it shall be inspected and

tagged by scaffolding supervisor and safety engineer before using it.

脚手架搭设完毕投入使用前，必须经工程技术员和项目部安全工程师检查确认挂牌，方能使用。

18.4 The plank of scaffolding shall meet the requirement and without crack.

The plank shall be fixed by iron wire. The plank extension out of the scaffolding shall not be allowed.

脚手架铺板强度必须保证安全使用，且无破裂/开裂。脚手架铺板必须用铁丝固定，不允许出现探头跳板。

18.5 Stair shall be installed for scaffolding. Rest platform shall be set

while the ladder is exceed 5m.

每个脚手架必须搭设好出入梯子，高于5M的梯子必须有一个休息平台。

18.6 Safety net shall be provided if there is any person working or passing

under the scaffolding to avoid injury by falling down object. 脚手架下面如有人作业或经过，应搭设安全防护层，防止落物伤人。

18.7 Warning tape shall be provided while scaffolding dismantling. The

scaffolding dismantling work shall be commanded and supervised by designated person.

脚手架拆除必须拉警戒线，有专人指挥、监护。

19 Radiographic Test 射线作业

19.1 “Radiographic Test Work Permit” shall be applied by radiographic

test worker. RT work can be carried out after application is approved. 由射线作业人员申请办理《射线作业许可证》，经批准后方可作业。

19.2 Radiographic test worker shall be allowed to execute RT work if he

has relative certificate.

射线作业实行持证上岗制度，没有取得相应资格证，不得进行作业。

19.3 RT work can be carried out after confirm nobody in the working area

except RT worker.

只有经检查确认射线作业区域内除作业人员外，已没有其它人员，才能进行射线作业。

19.4 Warning tape shall be provided for RT work area. Flash warning light

shall be set at each corner.

射线作业时，应在作业区域四周拉警戒线，在警戒线的每一个角应挂出闪光警示灯，以警告其它人员，危险作业正在进行中。

19.5 Monitor shall be used for RT worker to control the radiation dose

of worker.

射线作业人员应当配备监测器，以控制其暴露于辐射中的水平。

20 Fire Fighting Management 消防管理

20.1 Fire fighting work shall be in charged of authorized person.

认真贯彻落实消防安全责任制，做到“谁主管，谁负责”。

20.2 Emergency plan for fire fighting shall be prepared. Safety training

shall be performed for worker. Drilling shall be given if necessary. 制定火灾应急救援预案，组织对工人进行消防安全培训，定期进行演练。项目部成立防火领导小组，联络机构和应急小分队。

20.3 Circle access for fire fighting shall be provided at site. Material

cannot be placed on the access.

施工现场必须设置环行（消防）信道，禁止在信道上堆放物料，保持消防通道畅通。

20.4 Sufficient fire fighting equipment (fire extinguisher) shall be

provided for site, camp and office. And the fire fighting equipment shall be sensitive and effective. If it is necessary, well shall be arranged at site to provide the fire water while emergency. 应在施工现场、宿舍、办公室配备足够的消防器材(灭火器)，布局合理。保证消防器材灵敏、有效。现场还应根据需要打井，以便发生火灾时有消防用水。

20.5 If temporary building is required for construction, then it shall

meet the fire fighting requirement. Flammable material cannot be used for temporary building.

因施工需要搭建的临时建筑物，应符合防火要求，不得使用易燃材料。

20.6 Safety clarification shall be performed for painting work and

water-proofing work. And it can be carried out after relative countermeasure adopted.

油漆作业，防水施工等危险作业，要进行安全交底，采取具体的防火措施后方可作业。

20.7 Safety net and insulation material that used at site shall meet fire

fighting safety requirement. Flammable material cannot be used. 施工现场使用的安全网，密目式安全网，保温保冷材料，必须符合消防安全规定，不得使用易燃、可燃材料。

20.8 Watchman shall be provided for welding and gas welding work at height.

Flammable material around the working position or under the working position shall be cleaned before start the work. If it cannot be removed, then isolation measurement shall be adopted for the flammable material to avoid spark falling down on it.

高处电焊、气割作业，要设监火员。作业前清理周围及下方易燃物、可燃物，若不能清除，必须采取防止火花溅落到易燃物、可燃物上的隔离措施。

20.9 Hot work permit shall be applied before start the welding and gas

welding work within the flammable and explosive area.

在易燃、易爆等危险场所电焊、气割作业，必须办理动火证，方能作业。

20.10 Fire fighting safety inspection shall be executed in every three

months to get rid of the potential dangerous. 每个季度组织一次消防安全检查，消除消防安全隐患。

21 Housekeeping 文明施工

21.1 Site area shall be closed. 2.5m height fence shall be constructed

for site, camp and warehouse. Suitable greening shall be arranged for site office and camp according the actual condition. 施工现场实行封闭管理，工地、生活区、生产材料仓库采用高于2.5M的连续围墙进行封闭。工地办公室、生活区可因地制宜进行绿化。

21.2 Guard rules shall be complied for site. Nobody can be allowed enter

the site without card.

建立施工现场门卫制度，进入施工现场必须佩戴工作卡，无工作卡不得进入施工现场。

21.3 All worker shall wear the uniform while working at site.

进入施工现场，必须穿公司统一发放的工作服。

21.4 Helmet shall be used while enter the site. White helmet shall be used

for staff. Yellow helmet shall be used for worker. Red helmet shall be used for safety person.

进入施工现场必须正确戴好安全帽。安全帽颜色规定：管理人员为白色，工人为黄色，安全员为红色。

21.5 Brief introduce and safety slogan shall be set at the gate of site.

施工现场大门口处按规定设置五牌一图及安全标语。

21.6 Hardening treatment shall be performed for ground and access surface.

No water collection shall be allowed. Access for site transportation shall be prepared as per construction organization design. It shall

be arranged in circle, level, firm and unblocked. Spraying water shall be adopted at site for dust control.

施工场地、路面应进行硬化，排水畅通，无积水。施工运输道路按施工组织设计修建，要求平整、坚实、畅通，环形布置。施工现场应采取洒水降尘。

21.7 Smoking area shall be set at site. Smoking at non-smoking area shall

be strictly prohibited.

施工现场设置吸烟处，严禁在非吸烟处吸烟。

21.8 Construction material shall be stored at the designated area as per

construction layout. It cannot be placed anywhere. If the material shall be shifted to another place, then, it can be moved after application and approval.

施工材料、应按施工平面布置图指定的位置堆放，不得随意堆放。需要调整放置位置时应申请报批。

21.9 Temporary construction water and electrical shall be arranged as per

construction organization design. Light for working at night shall meet the safety requirement and security requirement.

施工用水用电必须按施工组织设计的规定进行敷设，不得乱拉乱接，夜间照明应满足施工安全和保卫要求。

21.10 Mud and sewage created during construction cannot be drained out side.

Sedimentation basin shall be provided for batching plant. 施工产生的泥浆、污水不得外流，混凝土搅拌站设有澄清池。

21.11 Toilet shall be arranged as per health requirement. It shall be

cleaned by designated person. Relieve oneself everywhere at site shall not be allowed.

厕所按规定设置，符合卫生要求，每天指定专人打扫卫生，施工现场不准随地大小便。

21.12 Canteen shall be kept clean and meet the health requirement. Worker

working in canteen shall be with health certificate. Rubbish shall be placed in dustbin at the designated position. Camp shall be cleaned everyday by designated person. Rubbish shall be cleaned everyday. 保持食堂清洁、卫生符合要求，工作人员经健康体检合格。生活垃圾堆放在指定地点，装入容器内每天清运。生活区每天派专人打扫卫生。

21.13 Bathroom shall be provided in camp and it shall be cleaned everyday.

生活区设置浴室，提供热水洗澡。浴室每天指定专人打扫卫生。

21.14 Potable water supply point shall be set at site to ensure the

acceptable potable water can be provided for worker. Housekeeping shall be done at site everyday for 10min before off-duty. 施工现场设有饮水点，保证施工人员饮用清洁卫生的水。施工现场每天下班前10分钟打扫清洁卫生，保持现场整洁。

21.15 First aid kit and first aid equipment shall be prepared at site.

现场配备急救箱和急救器材。

21.16 Gate security rules shall be established to avoid equipment and

material stolen.

建立治安门卫制度，现场设备材料不被偷盗。

21.17 Vehicle moving speed shall be limited within 15km/h at site.

施工现场车辆限速15公里/小时。

21.18 Several local labors shall be employed to do the housekeeping work in site.

长期雇佣几名当地人员每日对施工现场进行卫生清理。

22 Environment Protection 环境保护

22.1 Construction shall comply the requirement in Pakistan local law and

rules. De-dust and other countermeasure for waste material and water shall be considered in method statement.

遵守巴基斯坦当地政府有关环境保护的法律法规。在施工方案中应有防治

扬尘、固体废物和废水等污染环境的措施，并在施工中认真组织实施。

22.2 Separation tank shall be set for the waste water from canteen to

drainage pipe. It shall be cleaned by designated person to avoid pollution. A pit at proper position shall be arranged if no drainage near the outlet of waste water to avoid the waste water flow everywhere.

职工食堂废水凡接入下水道的必须设隔油物池，专人负责清掏，防止污染。附近无下水道的应选择适当地点挖渗坑，不得让污水横流。

22.3 Anti-leakage treatment shall be performed for warehouse that for oil

material storage to avoid soil and water polluted due oil material leakage.

现场存放油料，必须对库房进行防渗漏处理，防止油料泄漏，污染土壤、水体。

22.4 Construction rubbish shall be transferred to the designated area

timely. Rubbish placed at temporary area cannot more than 7 days. Effective countermeasure shall be adopted for rubbish transferring truck to avoid road pollution due to dust and rubbish falling. 施工垃圾及时清运到指定的场所，垃圾临时堆放不得超过7天。清运垃圾车辆应采取有效措施，防止运输途中扬尘和撒落污染道路。

23 HSE Audit HSE审计

Internal HSE audit shall be held in every half a year to evaluate the HSE management status and to make the HSE management under control. 项目部每半年将进行一次内部HSE审计，评价现场HSE管理状况，使HSE管理处于受控状态。

24 First Aid Point 急救站

A first aid station shall be set at site to provide the necessary first

aid equipment and to provide the necessary first aid to wounded person. 施工现场应建立一个急救站，并提供必要的装备，以便为伤者提供必要的救治。 One doctor shall be arranged from China with some necessary first aid medicine, first aid box and first aid litter. One vehicle shall be stand-by 24 hours for first aid. Moreover, CNCEC will contact with local hospital to get more support if necessary.

将从中国派遣一名专业医生，配备必要的急救药品、急救箱、急救担架，配备一辆值班车。此外，我们还将与当地医疗机构建立联系，以便必要时能得到他们的支持。

25 Traffic Rule 交通规则

27.1 CNCEC’s driver must comply the Pakistan traffic rule.

项目部机动车驾驶员必须遵守巴基斯坦交通法规。

27.2 CNCEC’s driver must has driving license.

项目部机动车驾驶员必须持有机动车驾驶证。

27.3 Vehicle shall be maintained as per vehicle instruction to ensure the

vehicle in good condition.

驾驶员应例行保养车辆，保持车况良好。

27.4 Speed limited 15km/h.

限速15公里/小时。

26 Accident Investigation & Treatment 事故调查处理

In case there is any accident occurred at ENGRO project site, it shall be investigated and treated as per CNCEC’s accident investigation and treatment procedure. Safety supervisor who is response for this area shall report to safety manager immediately after accident occurred. Safety manager shall arrange first aid after receiving the report, and meanwhile report the rough situation of accident to CNCEC project manager and ENGRO

safety manager. Then the investigation for accident will be organized by CNCEC safety manager. Accident investigation report shall be prepared as well as the punishment method, and then it will be submitted to CNCEC project manager for approval and it also shall be submitted to ENGRO safety manager.

在ENGRO项目施工现场发生事故，严格按照CNCEC规定的事故调查处理程序进行调查处理。在事故发生后，负责该区域安全的安全员应即向安全经理报告。安全经理在接到报告后尽快组织急救并向CNCEC项目经理和ENGRO的安全经理报告事故的大致情况。随后由安全经理组织进行事故调查。调查结束后整理事故调查报告并给出处理办法上报项目经理批准，同时将调查报告及处理办法报ENGRO安全经理。

29.1 Punishment Rules for Accident责任事故处罚规定

The punishment rules is applied to strengthen the safety management, to execute

the safety construction responsibilities rules, to avoid serious accident effectively, to define the responsibility of person who makes the accident. 为了加强安全管理，贯彻落实安全生产责任制，有效防范重大伤亡事故的发生，严肃追究事故责任者的责任，特制定本规定。

1 10 USD to 50 USD punish shall be given separately to the person who is full

responsible and major responsible and minor responsible for the minor accident.

发生轻伤事故，对负全部责任者、主要责任者、次要责任者，分别给予50美元至10美元的经济处罚。

2 While the serious injury accident occurred or the direct economic losses

more than 30 thousand RMB and less than 100 thousand RMB in one accident, 30 USD to 100 USD punish shall be given separately to the person who is full responsible and major responsible and minor responsible for the accident. 30 USD to 100 USD punish shall be given separately to the person among the project team management who is defined for the managing responsibility for the accident.

发生重伤事故或一次事故造成直接经济损失三万元以上、十万元以下的，对负全部责任者、主要责任者、次要责任者，分别给予100美元至30美元的经济处罚。项目经理部各级管理人员在事故中被认定负有领导责任或管理责任的，分级给予100美元至30美元的经济处罚。

3 While the death accident occurred or the direct economic losses more than

100 thousand RMB and less than 1 million RMB in one accident, 50 USD to 200 USD punish shall be given separately to the person who is full responsible and major responsible and minor responsible for the accident. 50 USD to 200 USD punish shall be given separately to the person among the project team management who is defined for the managing responsibility for the accident.

发生死亡事故或一次事故造成直接经济损失十万元以上、一百万以下的，对负全

部责任者、主要责任者、次要责任者分别给予200美元至50美元的经济处罚。项目经理部各级管理人员在事故中被认定负有领导责任或管理责任的，分别给予200美元至50美元经济处罚。

4 The subcontractor that safety accident occurred, it shall be punished as

per contract or agreement.

对发生安全责任事故的单位，执行合同（协议）上的处罚条款。 5 Punishment Execution Procedure 处罚执行程序

5.1

The punishment of personnel shall be deducted from current month salary by financial section after receive the official notice from safety manager.

对个人的处罚，由安全经理书面通知项目部财务部，由财务部从当月工资中扣除。

5.2

The punishment of subcontractor shall be deducted from current month payment by financial section after receive the official notice from safety manager.

对单位的处罚，由安全经理书面通知项目部财务部，由财务部从当月进度款中扣除。

29.2 Guard Rules 门卫制度

1 Guard must have obvious mark and must comply with their responsibilities.

门卫值班人员必须佩戴标志，忠于职责。

2 Visitor must have ID card and register at guard room.

外来人员须持有工作卡，在门卫室进行登记。

3 Everybody including worker and visitor shall be checked by guard while he

want move out of the site with any material or luggage. If anybody don’t comply the inspection, guard can detain his object and report to project team.

施工现场人员或外来人员携带物品、行李出门时，必须报告门卫，门卫有权进行检查，如不服从检查，门卫有权扣留物品并报告项目部领导。

4 Fighting, bust-up, ambling at site or keeping strangers in camp shall not

be allowed.

施工现场不准打架斗殴、聚众赌博，不得擅自留宿外来人员。 5 Anybody without helmet and card cannot enter the site.

未戴安全帽及工作卡标牌者严禁进入施工现场。

6 Vehicle shall be checked while it passes the gate. Un-necessary vehicle

cannot enter the site and camp. Vehicle with flammable, explosive and dangerous construction material can be allowed enter the site with valid certificate, but it cannot be allowed enter the camp. Gate pass is required for all material to be moved out of the side, and it shall be signed by project manager or construction manager and stamped.

车辆进出大门必须进行检查。与工程无关的车辆不得进入施工现场和生活区。易燃、易爆、危险物品必须持有有效证明，属于施工用品，方能进入施工现场，但不得进入生活区。从施工现场运出的所有物品，必须有出门条，出门条上有项目部印章和项目经理或施工经理签字。

7 由业主在人员进出口设置门卫室，门卫人员由业主提供。

EPCL shall set up guard room and provides the local guards.

29.3 Internal Security Rules 内部治安管理规定

1 Stealing material shall be strictly prohibited. 150 USD punishment shall

be given for the theft. The person that stolen large volume and with bad polt, he will be sent back to China.

严禁盗窃现场物资。凡盗窃现场物资者，罚款150美元。金额巨大、情节恶劣的，遣送回国。

2 Fighting and bust-up shall be strictly prohibited. 150 USD punishment shall

be given for those person that fighting and bust-up and compensated for all medical service. 150 USD punishment shall be given to person with major responsibility. 50 USD punishment shall be given to person with minor responsibility. Medical service shall be charged by themselves. The person with bad polt and ignoring the dissuasion shall be sent back to China. 严禁打架斗殴。无故殴打他人者，罚款150美元，赔偿全部医疗费用。凡相互打

架斗殴者，主要责任方罚款150美元，次要责任方罚款50美元。医疗费用双方按责任自行负担。情节恶劣，不听劝阻的，遣送回国。

3 Gambling shall be strictly prohibited. 150 USD punishment shall be given

to person join the gambling and ignoring the dissuasion. 严禁聚众赌博。凡参加赌博不听劝阻的，罚款150美元。 4 All playing shall be stopped by 12:00pm in camp.

宿舍晚12点应停止一切娱乐活动，熄灯就寝。

5 Everybody shall keep the valuables things and response for the lossing by

themselves.

宿舍内贵重物品自行保管好如有遗失，自行负责。

6 Take a good care of public property and public health.

爱护公共财物，爱护公共卫生。

29.4 Environment Health Management Rules 环境卫生管理制度

1 To make a good working environment, to keep a good working style, to improve

the CNCEC people’s health.

创造良好的工作环境，养成良好的文明施工作风，增进职工身体健康。 2 Living room shall be cleaned everyday and kept clean, health and in order.

职工宿舍整洁、卫生、整齐，每天打扫卫生。

3 Rubbish shall be placed at the designated area. Leftover cannot be poured

everywhere, it shall be poured into the dustbin. Bathroom and other common area shall be cleaned by designated person everyday. The proper countermeasure for heatstroke prevention and anti mosquito and housefly shall be adopted for camp and living room.

生产、生活垃圾堆放在指定的地方。剩菜剩饭不准乱倒，应倒入垃圾桶内。浴室及其它公共场所，指定专人每天打扫卫生，清除垃圾。生活区及宿舍要采取消暑和灭蚊蝇措施。

4 Construction rubbish shall be cleaned in every week.

施工现场建筑垃圾定期一周清除。

5 In case the communicable disease is discovered, it shall be reported to

project team timely.

如发现传染性疾病应立即向项目部领导报告。

6 Sanitary waste water cannot be drained everywhere, it shall be drained into

drainage pipe. Sewage pool shall be set for some place without drainage pipe.

生活废水不得乱倒乱流，应流入下水道。无下水道应设置污水池。

7 Toilet shall be cleaned everyday by designated person to keep the toilet

clean.

厕所设专人天天冲洗打扫，保持厕所清洁卫生。

8 Proper countermeasure such as medical or white pulverized lime shall be

applied in toilet periodic to get rid of housefly and maggot. 厕所灭蝇蛆措施，定期打药或撒白石灰粉，消灭蝇蛆。

29.5 Canteen Health Management Rules 食堂卫生管理制度

1 “Health Standard for Food” shall be strictly complied in canteen.

食堂应严格执行《食品卫生法》。

2 Do not sale or buy decomposed food. Canteen shall be apart from toilet more

than 30m.

不买不卖变质、变味、腐烂食品。食堂应离厕所30M以上。 3 Food and dishware shall be washed, cleaned and disinfected.

严格按照“一洗、二清、三消毒”认真清洁食品、餐具。

4 Cooked food shall be covered to prevent the housefly, miller and black

beetle.

熟食应用纱罩遮严，并注意防苍蝇、飞蛾、蟑螂。

5 Medical shall be used in canteen periodic to get rid of pest.

食堂应定期打药，消灭“四害”。

6 Health inspection by hospital shall be performed for person working in

canteen.

工作人员应进行健康体检，持证上岗。

7 Un-necessary person cannot be allowed enter canteen.

非工作人员禁止入内。

8 Waste water or dirty cannot be poured anywhere around the canteen. Rubbish

shall be sorted at the designated area and cleaned by designated person. 食堂周围不随意泼污水，扔污物，生活垃圾定点分类存放，每天专人清理。 9 Asbestos material cannot be used for canteen fitment.

不得使用石棉制品的建筑材料装修食堂。

10 Canteen shall be cleaned everyday and kept clean. Worker shall wear uniform

and pay attention to the personnel health. Chopping board shall be prepared separately for fresh food and cooked food.

食堂执行《食品卫生法》规定，设有有效的防蝇、防蟑螂、防鼠设施。食堂环境卫生每天打扫，保持整洁干净。工作人员穿工作服上岗，勤理发、勤洗澡、勤换衣、勤剪指甲。菜板生、熟食要分开。

29.6 Fire Prevention Rules 防火制度

2 Using fire and electrical furnace privately shall be strictly prohibited.

严禁私自生火和使用电炉。

3 Smoking on the bed and throwing the dog-eng in the room everywhere shall

be strictly prohibited.

严禁在宿舍内倒床抽烟，乱丢烟头。

4 Connecting the power cable without approval and using large power light

in the living room shall be strictly prohibited. The maximum power of light shall be not exceed 40W.

严禁在宿舍内乱拉电源线和点大灯泡，灯泡最大不得超过40W。

5 Flammable and explosive material cannot be stored in living room.

严禁在宿舍内堆放易燃易爆物品。

29.7 HSE Management Rules for Maintenance Work 检修工程施工安全管理规定

1 While hot work shall be carried out in maintenance area, flammable material

within this area shall be removed and the sufficient fire fighting equipment shall be provided.

在检修区域内动火，动火前必须清理干净周围易燃物，配备充足的消防器材。

2 No smoking within maintenance area.

施工人员在检修施工现场内严禁吸烟。

3 Playing during working shall be strictly prohibited.

遵章守纪，禁止在工作中打闹。

4 While maintenance for equipment or pipe that ever with flammable, explosive

and toxic medium, work permit shall be applied before enter the equipment or pipe. The content of flammable, explosive and toxic material cannot exceed the allowance value. Worker can enter the equipment or pipe after the effective safety countermeasure adopted and watchman provided. 凡检修可能存在易燃、易爆、易中毒物质的设备（管道），在进入设备（管道）前，必须按规定办理《作业票》。进入前经检测，其内的易燃、易爆、有毒及其他有害气体的浓度不能超过国家允许的标准，并采取有效的安全措施及派人监护后方可入内作业。

5 Worker shall master the performance and usage of gas mask.

作业人员必须熟悉防毒面具的性能和使用方法。

6 The proper PPE shall be used while contact the toxic and corrosive material

such as acid, alkali and ammonia etc..

接触有毒、有腐蚀性物质，如酸、碱、氨等，应按规定正确穿戴防护用品。 7 Working in the equipment or pipe or vessel that ever with toxic material

or acid, alkali and ammonia shall be carried out after replacement, cleaning and acceptance.

在有毒及酸、碱、氨等伤害的设备、管道、容器内部进行作业时，必须经置换清洗合格后方可作业。

29.8 Workers HSE Management Requirement 作业人员安全管理规定

1 Working at site after drinking shall be strictly prohibited.

严禁酒后进入施工现场作业；

2 Helmet and proper PPE shall be used while enter the site.

进入施工现场必须正确戴好安全帽，并正确使用个人劳动防护用品。

3 Enter site wear with short clothes, vest, skirt, high-heel shoe, sandal

and slipper shall not be allowed.

禁止穿短裤、背心、裙子、高跟鞋、凉鞋、拖鞋进入施工现场；

4 While working in metal vessel, safety voltage with good ground for anti

electrical shock shall be used. Portable light transformer cannot be taken into the metal vessel.

在金属容器内作业必须采用安全电压，行灯变压器严禁带入金属容器内； 5 No smoking shall be allowed at site and maintenance area. Smoking at smoking

area shall be allowed.

在施工现场、检修现场禁止吸烟，吸烟必须到指定吸烟室；

6 Safety insulation device, cover for opening and barricade for safety

warning board cannot be dismantled during construction.

在施工中不得任意拆除安全隔离装置、孔洞盖板、防护栏杆、安全警示牌； 7 Lifting machine shall be operated as per safety instruction.

Un-professional person cannot be allowed to operate the lifting machine. 起重机械损伤必须按起重机械安全损伤规程作业，严禁非专业人员操作起重机械；

8 Un-necessary person cannot be allowed enter the lifting area. Staying under

the boom or material shall be strictly prohibited.

起重吊装及危险作业区域非施工人员不得入内，严禁任何人在起重臂下及重物下方停留；

9 Flammable and explosive material such as gasoline cannot be poured at site

everywhere.

施工现场不得随意倾倒汽油等易燃易爆物品；

10 Driving after drinking and driving without license shall not be allowed.

严禁酒后驾驶机动车辆，严禁无证驾驶机动车辆；

11 Person seat at any position at outside of cab of moving crane, forklift

and dumper shall be strictly prohibited.

严禁乘坐运行中的吊车、铲车、翻斗车等驾驶室以外任何部位；

12 Playing during working shall be strictly prohibited.

严禁工作中打闹；

13 Special discipline worker with certificate can be allowed working.

特种作业人员必须持证上岗；

14 While many workers working together, attention shall be paid that do not

hurt yourself, do not hurt others and do not hurt by others.

工作时思想集中。多人作业时，必须互相注意安全，做到不伤害自己，不伤害别人，不被别人伤害；

15 Do not throwing any object from high position.

严禁从高处向下抛任何物体。

29.9 Countermeasure for Heatstroke Prevention 防暑降温管理办法

1 Patrol inspection at site shall be strengthened during hot weather to avoid

heatstroke.

为预防中暑，炎热季节施工，项目部要加强工地巡回检查。

2 Sufficient potable water shall be provided. A piece of towel shall be issued

to everybody in every three month.

保证提供足够的饮用水，按季度每人发放一张毛巾。

3 Some heatstroke prevention medicine shall be issued to staff.

项目部购买一些防暑降温类药品发给职工。

4 Working time shall be adjusted, overtime working shall be as few as

possible.

适当调整作息时间，尽量避免加班加点，做到劳逸结合。

5 Sunshade shall be provided for the working at opening area.

露天作业的职工应提供遮阳伞。
